Need to create user editable fields

We are trying to create fields which signer can add values to do it via PHP. is there any full documentation for that?

Hi Harishaidary, Thanks for your reply, Some how we managed and added all the fields but after signs the document, Nothing is happening, confirm menu is still disabled.
