Create a Template and Use it to Create a Package - .NET

I was asked a question about how to create a package from a template. Typically, if you're using the API/SDK, you can define your package in your class (essentially a template) vs using templates in eSignLive, but this question has come up several times, so I thought I'd make a simple example. In the downloaded zip file, you'll find a Word document with a step by step tutorial on creating your template and a .cs file that has code to take the template, create a package, and get a signing URL for one of the signers.
