What if you created a package out of this template from your own account, can you see the check boxes? Also, is it possible to share the template ID? It's in the route of the URL, in format of "https://sandbox.esignlive.com/a/template/LPjKz7TPmYxxxarp-8CU58="

I see the checkboxes when I'm setting up the template but when you send it out they don't appear.

Hi Lori,

I pulled the package JSON through support team and can create such a template with the same layout. When the package sent out, the signer 1 can see the three checkboxes on the second page. 

Please note that the second signer won't see the checkbox because the fields are not associated to him/her. And after selection, the check box only stamps a "X" to the document, without the border.
