Yes, it's completely possible to create your own consent form. To replace the default consent, you need to send the custom form to our support team so that all the subsequent templates and packages will be replaced with this new consent document.
In order to replace the consent document in your existing templates, you can use the "Add Consent Document Tool" I developed to do the job.
